通过 4 个简单的步骤，您就能在医疗保险市场参保 Carewell 批准的健康（医疗）保险计划，并准备好申请医疗保健费用援助福利所需的一切资料。如需申请医疗保健费用援助福利，请务必选择 Carewell 批准的计划。

第 1 步
在开始之前，请确保自己准备好以下信息：

* 重要事项：由于您的 SEIU 503 居家护理工作者谈判团队谈判争取来的 2026 年涨薪即将到来，您在 2026 年的收入可能会高于 2025 年。如果您的收入的确变高，请务必在医疗保险市场申请表中更新您的预估家庭收入。

登录您的医疗保险市场应用或在 HealthCare.gov 上创建账户，也可以致电 1-800-318-2596（全天候以多种语言提供服务）。
第 3 步
点击 “Check for all savings options”（检查所有优惠选项）方框，确保应用您有资格享受针对您保费的所有优惠。参保您所在县的 Carewell 批准计划。请在下方找到您的计划，并写下计划名称和计划 ID 编号，以确保您选择的是已获批准的计划：

请勿参保医疗保险市场牙科计划。所有有资格获得医疗保健费用援助的护理服务提供者都会自动加入 Carewell 牙科福利计划。
如需申请医疗保健费用援助福利，您需要向 Carewell SEIU 503 提交以下所需文件：

- 您的 Marketplace Eligibility Notice（医疗保险市场资格通知，从您的 HealthCare.gov 账户获得），以及
- My Plans & Programs（我的计划）页面（从您的 HealthCare.gov 账户获得）或您的保费账单（从您的承保公司获得）。
对于 从Providence 计划的参保者，请提交 “My plans & programs”（我的计划）页面。
申请 Carewell 批准计划的医疗保健费用援助有两种选择：
1.填写 Healthcare Cost Assistance Benefits Request Form（医疗保健费用援助福利申请表）并上传所需文件，或者
2.登录您的 MyCarewell503 账户并按照以下步骤操作：
- 在控制面板上，点击 Health Insurance Information（医疗保险信息）
- 接下来，点击 Update Health Insurance Information（更新医疗保险信息）
- 从下拉箭头中选择您的医疗保险类型
- 输入医疗保险生效日期（请勿输入结束日期）
- 从下拉菜单中选择医疗保险计划名称（承保公司名称将自动填充）
- 输入计划参与人数、保费金额、预付保费税收抵免额 (APTC)，系统将自动计算净保费
- 单击 “Choose file(s)”（选择文件）按钮上传您的 Marketplace Eligibility Notice（医疗保险市场资格通知）以及 “My plans & programs”（我的计划）页面或保费账单
在您参保医疗保险市场计划后，请确保在截止日期之前向您的承保公司支付首月保费。如果您是首次参保并有资格享受医疗保健费用援助福利，在您等待 Benefit Convenience Card（福利便利卡，BCC）到达期间，Carewell SEIU 503 会为您报销首月保费。
