Marketplace Enrollment Guide

Enroll directly on the Marketplace website at healthcare.gov or by calling 1-800-318-2596.

With 4 easy steps, you can enroll in a Carewell-approved Silver health (medical) insurance plan on the Marketplace and have everything you need to request Healthcare Cost Assistance benefits. To request Healthcare Cost Assistance benefits, you must choose a Carewell-approved Silver plan.

Please read this guide carefully to avoid common mistakes that may cause you to lose your medical coverage, your Healthcare Cost Assistance benefits, or both.

Step 1

Before you begin, make sure you have:

• • The email address you used with your Healthcare.gov account (if you already have one) or the email address you want to use.

  • Name, date of birth, and Social Security number for all people in your tax-filing household.

  • Estimated pre-tax (gross) income and deductions for all people in your tax-filing household. (Carewell has developed an optional Income Estimate Worksheet to help you estimate your 2025 income.)

  • If anyone in your tax-filing household has access to employer coverage, you will need the employer’s name, phone number, and the monthly cost of the plan.

Step 2

Log in to your Marketplace application or create an account with healthcare.gov or call 1-800-318-2596 (available in all languages 24/7).

Step 3

Click the “Check for all savings options” box and make sure to apply all the savings you’re eligible for. Enroll in the Carewell-approved Silver plan for your county. Find your plan below:

• • For plan resources, go to: CarewellSEIU503.org/en/plan-resources
  • Review the Oregon approved plans by county (PDF)
  • Review the Washington approved plans by county (PDF)
  • You can also choose your county from the dropdown list below. Write down the plan name and plan ID number so you’re sure to choose an approved plan.

Step 4

Do not enroll in a Marketplace dental plan. If you meet Carewell eligibility requirements, you are automatically enrolled in dental benefits.

To request Healthcare Cost Assistance benefits, you need to submit the following required documents to Carewell SEIU 503:

• • Your Marketplace Eligibility Notice (from your healthcare.gov account), and
  • My plans & programs page (from your healthcare.gov account) or your premium bill (from your insurance carrier).

(Providence enrollments, please submit the “My plans & programs” page.)

Options to request Healthcare Cost Assistance for a Carewell-approved plan:

1. Complete the Healthcare Cost Assistance Benefits Request Form and upload the required documents.

2. Log in to your MyCarewell503 account and follow these steps:

• Click on “Menu” in the top left corner.
• Choose “Member Profile.”
• Go to “Health Insurance Information.”
• Click on “Add another policy” (If you already have existing policy information in your account, click “Terminate this policy” first).
• Select the policy type, effective date, policy name, number of people on plan, premium amount, and Advance Premium Tax Credit (APTC).
• The net premium will be auto calculated.
• Do not enter the termination date, leave it blank.
• Click on “Choose file(s)” button to upload your Marketplace Eligibility Notice and your My plans & programs page or your premium bill.

3. Complete the Annual Paperwork. (Only if you want Valley Insurance Professionals as your Agent of Record: otherwise use Option 1 or Option 2 above.)

• On the last page, for the question “How would you like to enroll in your Marketplace plan?” — choose option 4, “My 2025 Marketplace enrollment is complete and I just need to submit my 2025 Annual Paperwork for Healthcare Cost Assistance.”
• Then 2 fields appear to upload/attach your required documents.
• Use these fields to upload the required documents (Marketplace Eligibility Notice and your premium bill or My plans & programs page).

After you enroll in your Marketplace plan, make sure to pay your first month’s premium to your insurance carrier before the due date. If you’re enrolling for the first time and are eligible for Healthcare Cost Assistance benefits, Carewell SEIU 503 reimburses you for the first premium while you wait for your Benefit Convenience Card (BCC) to arrive.
