How to access your Form 1099 online
Follow the steps below to get your Form 1099-MISC online.
Log in to your MyCarewell503 account
Link: MyCarewell503
Click on “Main Menu” in the top left-hand corner
Select “Forms and Documents”
Click “view/download” next to the 1099 Documents line
You will be redirected to Zenwork where you will be able to view/download your document.
Enter your username and password
Your username will be the same email address that you use to sign in to MyCarewell503.
Your password will likely be:
- The first 4 letters of your first name and the last 4 digits of your Social Security number OR
- The first 4 letters of your last name and the last 4 digits of your Social Security number.
