Benefit Summary
The Carewell SEIU 503 Dental benefit provides coverage that pays for most basic dental services at no cost to you and also pays a portion of other more complex dental services.
Carewell SEIU 503 Dental benefits are insured by Kaiser Permanente.

Important Things to Know
- This coverage is dental insurance only, not medical insurance.
- Once you are eligible, you will have coverage through a Kaiser Permanente dental plan. The only thing you may need to do is update Carewell SEIU 503 and the State with your name, gender, Social Security number, and current address.
- The Carewell SEIU 503 Dental benefit is premium-free — there is no monthly bill for you to pay.
- You cannot use the Benefit Convenience Card to pay for any expense related to your dental plan.

Eligibility Requirements
To be eligible for Carewell SEIU 503 Dental benefits you must:
You must work at least 40 eligible working hours* for 2 months in a row to be eligible for these benefits. Once you have met this requirement, there will be a 1-month waiting period after you turn in your timesheets and/or payroll vouchers. During this waiting period, the State, Acumen, or PPL will report your hours to Carewell SEIU 503.
Turn in your timesheets and/or payroll vouchers on a regular basis.
Have your name, gender, Social Security number, birth date, and current address on file with Carewell SEIU 503. Click here to update your information now.
These eligibility requirements are the same for the Dental, Vision + Hearing, and Employee Assistance Program (DVE) benefits.
* Eligible working hours
The number of eligible working hours you need to qualify for Carewell benefits are the work hours paid by Acumen, DHS, and/or PPL on behalf of your consumer.
Losing eligibility
You may lose eligibility for Carewell SEIU 503 benefits (Dental, Vision + Hearing, EAP, and Healthcare Cost Assistance) if you have 2 months in a row with 0 hours of work. After those 2 months, you’ll have a grace period of 1 more month before your benefits end.
You will be notified by mail that you might be losing your benefits. These notifications also include information about continuing Dental, Vision and Hearing, and EAP coverage through COBRA.
If you decide to stay on DVE benefits through COBRA, this means you will now have to pay for premiums by sending payment to our COBRA Administrator, Ameriflex.
You can also regain eligibility by meeting the requirements listed above.
Enrollment
Dental, Vision + Hearing, and Employee Assistance Program benefits will be set up automatically for you once you:
- meet the Eligibility Requirements explained above;
- update Carewell SEIU 503 and the State with your name, gender, Social Security number, and current address.
Enrollment Confirmation
If your medical insurance is through Kaiser
You will not get a separate Kaiser Permanente dental card if you live in the Kaiser service area and you currently have Kaiser Permanente medical insurance. You will have one card for both medical and dental insurance.
Inside the Kaiser service area (HMO plan)
If your medical insurance is not through Kaiser
You will receive a Kaiser Permanente dental card in the mail if you do not have medical insurance with Kaiser. Note that this card is only for dental insurance and not for any other benefits.
Outside the Kaiser service area (PPO plan)

Be sure to confirm that your provider is in-network before seeking services and carefully review the benefit summary for information about possible out-of-pocket expenses like copays and coinsurances. Carewell SEIU 503 does not reimburse you for additional costs and you cannot use the Benefit Convenience Card for dental expenses, so check with your provider about procedures that are not fully covered.

Keep This Benefit
You do not need to reapply or re-enroll as long as you do not go 2 months in a row without working — or having 0 hours reported, you will remain eligible for Dental, Vision + Hearing, and Employee Assistance Program benefits. You will not receive a new card every year.

Do I have options other than Kaiser Permanente Dental?
Carewell SEIU 503 does not offer other options for your Dental coverage. However, if you live outside a Kaiser service area, you’ll be enrolled in Kaiser Permanente’s Dental Choice PPO plan. Under the KP Dental Choice PPO, you will be able to visit any licensed dentist. However, you will pay less if you visit a dentist that participates in Kaiser’s PPO network. To confirm whether you live inside or outside a Kaiser service area, contact Carewell SEIU 503 at 1-844-503-7348.
I already have a dental plan. Can I have two?

I already have a dentist that I like. Can I keep going to this dentist?
If you reside outside a Kaiser service area and have a specific dentist who is not in-network:
Ask your dentist if they’d be willing to get credentialed with Kaiser Permanente so they can become part of the network.
If you reside inside a Kaiser service area you and have a specific dentist who is not in-network:
You will not receive coverage for any services performed by an out-of-network dentist.
Can I add my spouse or other family members?
No. Carewell SEIU 503 benefits are available only to eligible homecare and personal support workers and personal care attendants.
